Responsibilities

  • Provide compassionate and dignified care to patients in their homes or hospice settings.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Document patient progress and communicate effectively with patients, families, and other healthcare providers.
  • Adhere to all regulatory standards and maintain compliance with healthcare policies and procedures.

Requirements

  • Valid state license to practice as a registered nurse (RN).
  • Bachelor’s degree in Nursing or related field.
  • At least one year of relevant clinical experience preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Qualifications

  • Compassion and empathy for those facing illness or end-of-life challenges.
  • Strong organizational and problem-solving skills.
  • Proficiency in Microsoft Office Suite.
