Nurse Practitioner - Hospice
About the Role
Works in collaboration with physicians and hospice interdisciplinary team members to deliver palliative care services. Provides home visits for face-to-face assessments, GIP (general in-patient) consults/admissions, on-call provider coverage, and comfort care consults. Provides patient and family education, psychological support, social support, and survivorship care planning throughout the continuum of care. Works with patients and families to establish goals of care related to health care services. Assists with staff development.
Requirements
- Master’s degree in Adult Health Nursing/Nurse Practitioner
- Minimum two years’ experience in a palliative care setting
- Current Louisiana APRN license
- Current Basic Life Support (BLS) certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses (e.g., Ochsner Health System, American Heart Association, American Red Cross). The ILCOR course must include a manikin check-off.
- Excellent interpersonal and communication skills
- Knowledgeable on CMS guidelines for Hospice
- Valid and current state-issued driver’s license
- Must maintain and provide proof of automobile insurance (at own expense) meeting at least the minimum liability coverage limits required by the state of residence
Physical Demands
- Must possess good physical health
- Standing, sitting, or walking for long periods of time
- Lifting at least 20 pounds is required
- Must be able to work with a moderate level of noise

- Constant (67%-100%): handling/feeling, talking, hearing, seeing
- Frequently (34%-66%): lifting, carrying, pushing/pulling, stooping, crouching, reaching
- Occasionally (1%-33%): climbing (stairs, ladders, etc.), crawling
