Pay

  • $125 per visit
  • $75 admin rate

Responsibilities

  • Communicate significant findings, problems, changes in condition, or environment to the clinical manager, attending physician, and interdisciplinary team
  • Complete face-to-face encounters to assess disease-related changes, decline in function, adherence and effectiveness of pain and symptom regimen, patient and family coping mechanisms, and quality of life
  • Order and review appropriate lab tests, x-rays, EKGs, and other diagnostic procedures for palliation and treatment of terminal disease; document the treatment plan and review results with collaborating physician
  • Make referrals to other disciplines as indicated by patient needs
  • Actively participate in interdisciplinary team conferences

Qualifications

  • Current and unencumbered Nurse Practitioner license in the state of employment
  • Minimum of one (1) year experience as a Nurse Practitioner or one (1) year experience as a hospice Registered Nurse in the last three (3) years
  • Authority to practice and prescribe as a Nurse Practitioner in the state where services are provided
  • Medicare billing privileges as a Nurse Practitioner
  • Meet state-specific physician-Nurse Practitioner collaborative requirements; supply a copy of all active collaborating physician agreements and indicate transferability to Amedisys
  • Insurance required by state law for Nurse Practitioners to practice and prescribe, or eligibility to satisfy requirements through group coverage with Amedisys
  • Knowledge of Medicare reimbursement and coding for all levels of service (home, office, nursing home, assisted living, etc.), Medicaid, and other regulatory requirements
  • Current CPR certification
