About the role
Hospice Nurse Practitioner responsible for providing palliative, symptom management, and supportive care to hospice patients.
Responsibilities
- Conduct face-to-face visits with hospice patients, reporting findings to the Hospice Medical Director
- Collaborate with physicians to develop and coordinate individualized care plans
- Document patient conditions, goals, interventions, and responses, ensuring accurate clinical notes
- Educate and supervise patients and families in self-care techniques when appropriate
- Communicate patient needs and issues to the healthcare team and administrative staff
Qualifications & Skills
- Unrestricted active license and current certification as a Nurse Practitioner or Clinical Nurse Specialist
- Certification from a recognized national body (ANCC, NBCHPN, AANP)
- CPR certification and minimum of 5 years nursing experience
- Valid driver's license, access to transportation or public transit
Preferred Experience
- Working with interdisciplinary teams
- Clinical care in oncology, hospice, geriatrics, or intensive care
- Palliative care ARNP experience
