Nurse Practitioner - Hospice

About the role

Hospice Nurse Practitioner responsible for providing palliative, symptom management, and supportive care to hospice patients.

Responsibilities

  • Conduct face-to-face visits with hospice patients, reporting findings to the Hospice Medical Director
  • Collaborate with physicians to develop and coordinate individualized care plans
  • Document patient conditions, goals, interventions, and responses, ensuring accurate clinical notes
  • Educate and supervise patients and families in self-care techniques when appropriate
  • Communicate patient needs and issues to the healthcare team and administrative staff

Qualifications & Skills

  • Unrestricted active license and current certification as a Nurse Practitioner or Clinical Nurse Specialist
  • Certification from a recognized national body (ANCC, NBCHPN, AANP)
  • CPR certification and minimum of 5 years nursing experience
  • Valid driver's license, access to transportation or public transit

Preferred Experience

  • Working with interdisciplinary teams
  • Clinical care in oncology, hospice, geriatrics, or intensive care
  • Palliative care ARNP experience
