Nurse Practitioner - Home Based Primary Care

About the role

The Nurse Practitioner will deliver comprehensive primary care, palliative care, and chronic disease management to patients in their homes. Responsibilities include conducting physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers.

Responsibilities

• Deliver comprehensive primary care, palliative care, and chronic disease management to patients in their homes.
• Conduct physical examinations, diagnose and treat illnesses, manage chronic conditions.
• Engage in advanced care planning discussions.
• Collaborate with the interdisciplinary team and communicate with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations.
• Participate in on-call coverage for the Integrated Health Services population as assigned.

Requirements

• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS).
• Full-time position; Monday - Friday 8:00am - 5:00pm (office base will be High Point); on-call responsibility.
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Qualifications

• Master's degree in Nursing (MSN) or Doctorate in Nursing Practice (DNP).
• Current licensure as a Nurse Practitioner in North Carolina.
• Experience in home-based primary care preferred.

Skills

• Ability to build and maintain therapeutic relationships with patients and caregivers.
• Proficiency in health promotion, disease prevention, and patient education.
