Responsibilities

• Provide comprehensive in-home healthcare to adult and senior patients.
• Perform health assessments and manage both chronic and acute conditions.
• Prescribe medications and coordinate care with physicians and home health teams.
• Educate patients and families on health management and preventive care.
• Document patient interactions and care plans in an Electronic Health Record (EHR).

Requirements

• Valid Nurse Practitioner license with primary care certification.
• Experience in home health, primary care, or related settings preferred.
• Strong clinical assessment and management skills.
• Excellent communication and patient education abilities.
• Ability to collaborate effectively with multidisciplinary teams.
