About the role

The Nurse Practitioner (NP) provides advanced clinical expertise in geriatrics and palliative care as a key member of Sharp Memorial Hospital’s Generational Health Program. This program is dedicated to improving the care of older adults by supporting function, dignity, independence, and quality of life throughout the healthcare continuum. Through a collaborative, interdisciplinary approach, the NP partners with multiple inpatient service lines, physicians and surgeons, nursing, pharmacy, case management and social work, therapy and rehabilitation, specialty services, and community resources to develop individualized, patient-centered care plans that align with each patient's goals, values, and clinical needs. The NP plays a critical role in identifying and addressing age-related vulnerabilities, supporting complex decision-making, managing serious illness, and integrating evidence-based, age-friendly care principles. Working directly with patients, families, and caregivers, the NP promotes optimal outcomes through comprehensive assessment and treatment including surgical evaluation and mitigation, symptom management, advance care planning and goals of care, and care coordination. Sharp Memorial Hospital is nationally recognized for excellence in the care of older adults and has achieved multiple national distinctions, including Geriatric Emergency Department Accreditation, American College of Surgeons Trauma Verification, American College of Surgeons Geriatric Surgery Verification, and Age-Friendly Health System Recognition. The NP contributes directly to the continued success of these initiatives by advancing clinical excellence, quality outcomes, and innovative models with a team-based, collaborative approach.
