Responsibilities
- Hospital Medicine Responsibilities:
- Conduct comprehensive evaluations and assessments for hospitalized patients.
- Develop and implement individualized treatment plans.
- Provide medication management and monitor patient responses.
- Collaborate with physicians, nurses, and multidisciplinary teams to ensure coordinated patient care.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Nursing Home / Skilled Nursing Facility (SNF) Responsibilities:
- Provide routine, acute, and transitional care for patients in nursing home and skilled nursing settings.
- Perform initial and follow-up visits, including post-hospital discharge evaluations.
- Manage chronic conditions and acute changes in patient status.
- Cook up care plans with nursing staff, therapy teams, and facility administration.
- Conduct medication reconciliation and adjust treatments as appropriate.
- Participate in care plan meetings and communicate regularly with patients’ families as needed.
- Collaborate with hospital teams to support smooth transitions of care between the hospital and nursing facilities.

- Affiliated Nursing Homes and Skilled Nursing Facilities throughout the surrounding service area
