Nurse Practitioner - Fee for Service, $5,000 Sign On Bonus

About the Role

TruHealth is seeking an advanced practice provider—either a Nurse Practitioner (NP) or Physician Assistant (PA)—with experience in primary care or internal medicine, preferably with a geriatrics focus. This position offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. You’ll work primarily within nursing homes or assisted living communities, delivering personalized, coordinated health care that enhances patient outcomes and prevents rehospitalization.

Responsibilities

• Assess patients’ medical and mental health needs and develop proactive care plans that address conditions and barriers to care.
• Ensure compliance with local, state, and federal regulations related to clinical services provided.
• Prescribe medications, order lab work, diagnostic procedures, and consultations.
• Monitor patient compliance and response to treatment, modifying plans as needed.
• Implement TruHealth’s Treat in Place Protocols in collaboration with facility partners and health plan leadership.
• Consult with supervising physicians as appropriate while exercising independent judgment in treating acute and chronic conditions.
• May provide training and advice to facility staff as a supervisory responsibility.

Requirements

• Active APRN or PA license required.
• Degree from an accredited APRN or PA program.
• Minimum of 3 years’ experience in clinical nursing, rehabilitation, or geriatric populations, and/or fee-for-service experience.
• Proficiency with Electronic Health Records (EHR) systems.
• Working knowledge of Microsoft applications, including Word, Outlook, and Excel.
