About the Role

The Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise. The primary responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education.

Responsibilities

• Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, family history, psychosocial situation, cultural factors, and performing appropriate physical examination.
• Order/perform appropriate laboratory diagnostic and other screening tests. Seek additional information as needed, including consultation with physicians and other clinicians, for evaluation of illness.
• Integrate data to determine diagnosis and therapeutic plan, including identification of any health risks.
• Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Educate patients regarding medications, treatments, health promotion, and illness prevention.
• Recommend appropriate community resources to meet patient/family needs. Communicate case management information to other professionals and community agencies.
• Prepare documentation for EMR, including updating patient records with examination results, diagnosis, medications, and treatment.
• Participate in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities.
• Act as consultant/collaborator to clinical team on area of specialty. Comply with patient confidentiality requirements and promote patient advocacy.
• Collaborate with physicians in managing acute and long-term medical needs of patients. Provide monitoring and continuity of care between visits, including triaging patient calls/emails.
• Perform all other duties as required or assigned.

Assessment

• Elicit and document a complete health history.
• Perform and document a complete physical examination.
• Order appropriate diagnostic tests.
• Assess patient’s perception of their health status, personal strengths, support systems, and patient problems, including psycho-social, physical, environmental, economic, and cultural factors.
• Identify patients and families at risk for specific healthcare problems.

Diagnosis and Planning

• Formulate diagnoses based on health history, physical examination, and diagnostic tests.
• Formulate and document health care plans with patients and their families, including treatment, health maintenance education, and counseling.
• Establish, review, and document short and long-term goals in collaboration with each patient.
• Plan comprehensive patient care with other healthcare providers.

Implementation

• Demonstrate ability to provide appropriate care based on needs of a specific individual, including age-specific considerations.
• Institute treatment plans based on assessments and interpretations of diagnostic tests and physical findings.
• Counsel patients according to their individual motivation and compliance.
• Provide telephone advice and triage for acute and unscheduled patients.
• Document all pertinent data and interventions in the patient’s records.
• Provide refills for routine medications.
• Institute appropriate action when reviewing abnormal lab data and diagnostic tests.
• Consult with other providers, as indicated.

Evaluation

• Evaluate the patient’s understanding of and compliance with the healthcare plan through regular follow-up and reassessment.
• Re-evaluate short and long-term goals with patients and participating health professionals.

Education

Patient and Family

• Identify the learning needs of patients and family members.
• Assess the patient’s readiness to learn.
• Provide individualized health education.
• Develop teaching goals and objectives that meet the patient’s and family’s healthcare needs.

Staff

• Function as a clinical resource person.

Continuity/Coordination

• Function as the healthcare provider for a specified group of patients.
• Coordinate patient care activities and healthcare planning among multiple providers, patients, and their families.
• Communicate and consult with family, staff, and unit personnel prior to and during the patient’s hospitalization. May participate in discharge planning when appropriate.
• Promote ongoing communication with community agencies to facilitate continuity of patient care.

Advocacy

• Promote the participation of patients and families in planning their care.
• Assist patients in obtaining information necessary to make informed decisions and support patients in decision-making.
• Inform patients of the Patient Bill of Rights.
• Provide referrals to community resources when appropriate.

Requirements

Qualifications – Nurse Practitioner

• BSN required; MSN preferred.
• Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program.

License/Certifications

• Current Registered Nurse license, as defined by the California Board of Registered Nursing.
• Active and unrestricted Nurse Practitioner license in the state of California.
• Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable).
• Must hold current national board certification by the first day of work and maintain it.
• Must hold an active and transferable DEA license to California as required by the clinical specialty.
• Must be eligible for payer enrollment with CMS Medicare and Medicaid.
• BLS Certification (American Heart Association) required.

Experience

• 2 years of experience in advanced practice or specialty preferred.
• 2 years in specialty area highly desired.
• Prior experience in a medical office environment preferred.

Pay

The pay range for this role is $85.95 - $100.27 - $117.50 per hour, complying with applicable law. Individual pay is set by skills, experience, qualifications, and internal/market equity, consistent with MarinHealth’s compensation philosophy. Positions covered by collective bargaining agreements are governed by those agreements.

Schedule

This is a full-time position with scheduled weekly hours of 40, working 8-hour day shifts.
