About the role

As the Nurse Practitioner (CRNP) - Hospice, you will support the Redstone mission by providing palliative and end-of-life care to hospice patients. This is a casual position with varying days and shifts as needed.

Responsibilities

  • Conduct face-to-face hospice visits to assess and document patients' continued eligibility for the Medicare Hospice Benefit.
  • Perform comprehensive patient assessments and provide palliative care interventions to support comfort and quality of life.
  • Collaborate with the Hospice Medical Director, attending physicians, and interdisciplinary team to coordinate individualized patient care.
  • Document patient visits, clinical findings, and required hospice recertification information accurately and timely.
  • Provide consultation and clinical support regarding palliative and end-of-life care while maintaining compliance with Medicare and regulatory requirements.
  • Promote compassionate, patient-centered care while supporting the mission and philosophy of Redstone Hospice.

Requirements

  • Current Pennsylvania Certified Registered Nurse Practitioner (CRNP) license and national certification.
  • Master’s degree or higher from an accredited Nurse Practitioner program.
  • Minimum of 2 years of Nurse Practitioner experience.
  • Understanding of hospice and palliative care principles preferred.
  • Valid Pennsylvania driver’s license and reliable transportation.
