Nurse Practitioner - Chronic Care

Role Overview This position involves providing advanced nursing care by assessing, planning, and implementing patient health strategies. It includes both direct and indirect nursing responsibilities, guiding and evaluating care delivered by team members, and serving as a resource for advanced nursing expertise. The role emphasizes education, counseling, and collaboration with patients and healthcare teams to promote health and prevent illness. Key Responsibilities Conduct comprehensive health assessments and interpret diagnostic data.Diagnose health conditions and oversee patient care plans.Supervise and coordinate nursing, medical, and support staff.Develop individualized patient education and health promotion programs.Maintain accurate documentation and legal records.Engage in research, policy development, and system improvement efforts.Participate in ongoing evaluation of healthcare services and programs.Perform patient care as needed to maintain clinical competence and support team functions. Qualifications & Skills Master’s degree in nursing with relevant specialty focus.Licensed Advanced Practice Registered Nurse and Registered Nurse in Hawaii.National Provider Identifier and Basic Life Support certification.Certification as a Nurse Practitioner.Strong customer service, communication, problem-solving, and teamwork skills.Experience in specialty area and familiarity with PC applications is preferred.
