Nurse Practitioner (Cantonese Required)

We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable, and value-based primary care to adult Medicare patients. In this role, you will independently see patients, conduct office visits for routine and acute issues, and perform wellness visits and health assessments. You will coordinate care with other providers, assist with phone triage, educate patients, and build long-term relationships to foster trust and understanding. Working within a collaborative outpatient team, you will partner with physicians, nurses, medical assistants, and specialists to focus on primary care, care coordination, and population health management. Qualifications Include Master’s or Doctorate in Nursing or NP certification Active, non-probationary NP license and DEA license Certification in Family, Adult-Gerontology, or Gerontological specialties US work authorization Preferred Skills Primary care experience in internal medicine, geriatrics, or family medicine Knowledge of HCC documentation, ICD-10 coding, and health risk assessments Team-oriented mindset and passion for innovative population health models Bilingual abilities are a plus.
