Nurse Practitioner (Cantonese Required)

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities: Provide effective, equitable primary care to adult Medicare patients, including routine and acute visits, annual wellness exams, health risk assessments, and care coordination. Build trusting patient relationships, educate patients on health conditions, participate in population health initiatives, assist with phone triage and outreach, conduct home visits if needed, and collaborate with a multidisciplinary care team and specialists. Qualifications: Master of Science in Nursing, Doctor of Nursing Practice, or NP certification; active, non-probationary NP license; active DEA license; US work authorization. Preferred experience in primary care (internal medicine, geriatrics, family medicine), understanding of HCC documentation, ICD-10 coding, and health risk assessments; bilingual skills are a plus. Skills & Attributes: Strong teamwork, patient education, population health focus, and cross-functional collaboration. Ability to work independently and build meaningful patient relationships. Details: Full-time role with approximately 40 hours weekly. Compensation ranges from $95,738 to $206,206 annually, depending on experience and location.
