Essential Functions
- Directs medical assistants, care managers, nurses, and care coordinators in order to provide efficient and effective patient centered care, as described in the team based policy.
- Takes full patient histories and performs examinations to develop and implement a plan based on diagnostic and demographically specific evidence based guidelines.
- Orders and reviews diagnostic labs and imaging; orders and reviews consultations; administers procedures including shots, as needed; communicates with patients and caregivers regarding their individualized care management plan; collaborates with internal and external care providers; provides appropriate healthcare instruction and education; documents all healthcare communication and action in the electronic medical record.
- Engages in pre-visit planning with the care team to review and plan for the day’s patient panel and their specific care needs.
- Trips all patients requesting service in a timely manner regardless of the medium.
- Responds to emergencies as needed.
- Provides backup coverage to other provider panels as needed.
- Provides extended hours care, as well as after hour's clinical advice as determined by the Adult Health Services schedule.
- Follows office policy and procedures and demonstrates knowledge of the requirements of a NCQA Patient Centered Medical Home.
Care Management
- Collaborates with the care manager in stratifying primary care patients according to medical, behavioral, and social determinants of health using population management tools in the electronic health record.
- Stratifies patients according to the number and type of internal/external referrals and their overall healthcare utilization to identify high risk primary care patients in need of care management plans.
- Develops and executes individualized care management plan using comprehensive health assessments with the care team to provide appropriate patient centered care that meets the needs and goals of the patient.
- Consistently revalues the individualized care plan and a patient’s progress towards their goals and assesses barriers when objectives are not met.
Quality Improvement
- Participates in clinical quality initiatives that are based on State or Federal programs (i.e., Quality Payment Program, MIPS, MU), as well as organizational initiatives.
- Agree to data transparency, as well as the sharing of and accountability for clinical quality metrics.
- Participates in quality improvement meetings and activities as needed.
- Attends all required medical home and Adult Health Services meetings.
Qualifications
- Education Level: Graduation from an approved Nursing Program with a Master’s degree in a relevant area of specialization (i.e. Family Practice); Possession of a license to practice medicine in New York State; Board Certified Family Nurse Practitioner.
- Knowledge of: Electronic medical records; Awareness of current local, state, and federal laws governing the delivery of care.
