Your Impact

• Deliver field-based care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient's home.
• Manage chronic disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate across the continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

What You Bring

• Advanced Degree: Master's Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
• West Virginia Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable Mobility: Valid driver's license and vehicle for local field-based travel.

Location: Parkersburg, WV (Field-Based / In-Home)
Salary Range: $99,524 - $158,413 + Mileage & Full Benefits - Relocation available!

• Drug Screening Requirement: As a condition of employment, WellBe Senior Medical requires all candidates to successfully complete a pre-employment drug screening.
