Responsibilities
- Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in patients’ homes.
- Manage Chronic Disease: Develop longitudinal care plans for high-risk patients.
- Collaborate Across the Continuum: Partner with an interdisciplinary team for seamless care transitions.
- Prevent ER Visits: Treat acute needs in the home to reduce hospitalizations for seniors.
Requirements
- Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
- OH Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
- Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
Pay
$104,763 - $166,750 + mileage reimbursement. Relocation assistance available.
