Nurse Practitioner

About the role

Ultra Health Providers offers a full-time position for a Nurse Practitioner (NP) to provide primary and preventive care to residents in skilled nursing and assisted living facilities. The role includes managing acute and chronic conditions, conducting physical exams, medication reviews, and depression screenings. The NP will work closely with facility administrators and staff to ensure exceptional patient outcomes.

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
• Conduct admission assessments, physical exams, medication reviews, and depression screenings
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
• Communicate with and counsel family members on treatment options and changes in health status
• Manage clinical documentation accurately and in compliance with facility and state standards

Qualifications

• Active and unrestricted Nurse Practitioner license in Tennessee
• Current national board certification (AANP or ANCC)
• Minimum 1-2 years of experience in primary care, geriatric, or long-term care settings preferred
• Strong clinical judgment and communication skills
• Commitment to compassionate, patient-centered care
