Nurse Practitioner

As a Nurse Practitioner, you will serve as a trusted clinician for residents in skilled nursing and assisted living facilities, managing acute and chronic conditions, providing preventive care, and partnering closely with nursing staff and administrators to ensure exceptional patient outcomes. The role offers a part‑time, Monday‑Friday schedule and the opportunity to make a difference in the lives of seniors.

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
• Conduct admission assessments, physical exams, medication reviews, and depression screenings
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
• Counsel patients and family members on treatment options and changes in health status
• Manage clinical documentation accurately and in compliance with facility and state standards

Qualifications

• Active and unrestricted Nurse Practitioner license in Tennessee
• Current national board certification (AANP or ANCC)
• Minimum 1-2 years of experience in primary care, geriatric, or long-term care settings preferred
