About the role
Serve vulnerable populations through high-presence, quality care within skilled nursing and senior living settings.
Responsibilities
- Provide primary, urgent, and preventive medical care to residents
- Independently evaluate, diagnose, and treat acute and chronic medical conditions
- Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings
- Maintain accurate and timely documentation in the electronic medical record
- Collaborate with interdisciplinary teams to optimize resident outcomes
- Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition
- Adhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice
Requirements
- Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice
- Current national Nurse Practitioner certification (ANCC, AANP, or equivalent)
- Clinical training and experience in adult, family, and/or geriatric medicine
- Eligibility to work in the United States
Qualifications
- Experience in internal medicine, geriatrics, cardiology, or post-acute care
- Active DEA registration or ability to obtain
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
- Organized, detail-oriented, and able to manage multiple priorities effectively
- Prior experience in SNF, LTC, ICU, or Emergency Department settings is a plus

Pay
$100,000 – $125,000 annually (based on experience)
