About the role
The HealthCare Connection (THCC) is seeking a dedicated and experienced Nurse Practitioner (NP) to join their clinical team. This role works as part of the clinical provider team, delivering a wide range of direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans.
Responsibilities
- Provide high-quality patient care through the diagnosis and treatment of acute and chronic conditions
- Obtain and review patient histories (medical, developmental, social, and family) and perform comprehensive physical exams
- Order, perform, and interpret diagnostic tests and preventive screenings
- Develop and manage treatment plans, including prescribing medications and educating patients on conditions and care
- Promote preventive care through immunizations, screenings, and patient education on health, nutrition, safety, and disease prevention
- Document all patient encounters accurately, including history, exam findings, diagnoses, and care plans
- Review and maintain patient records, including labs, imaging, medications, and immunization status
- Coordinate care by facilitating referrals and collaborating with healthcare team members
- Monitor patient progress, follow up on test results, and manage high-risk patient population
Qualifications
- Education: Master’s or Doctoral degree in Nursing
- Certified Nurse Practitioner licensed in the State of Ohio
- Work Experience: Minimum of 5 years of experience in family medical practice
- FNP preferred, with a strong desire to work with children
- Experience in a physician office, ambulatory care, or clinical health setting preferred
- Qualifications: Board certification as a Nurse Practitioner
- Active DEA license (or ability to obtain)
- Strong clinical decision-making and communication skills
- Commitment to patient-centered care
Preferred Qualifications
- Experience in primary care, family medicine, or community health
- Bilingual skills (if applicable)
- Familiarity with electronic health record (EHR) systems
