Nurse Practitioner

About the role

The Hospice and Palliative Care Nurse Practitioner (NP) at Providence at Home with Compassus is a key member of the interdisciplinary team providing comprehensive care to patients and families in various care settings. The NP functions independently, coordinating and assessing care needs, and collaborating with the interdisciplinary team to ensure high-quality care.

Responsibilities

• Complies with organization-wide policies and procedures, including regulatory requirements.
• Conducts thorough medical, functional, and psychosocial assessments on assigned patients.
• Performs and interprets diagnostic tests and procedures as defined by protocol.
• Provides appropriate medical intervention when needed, including writing orders according to established protocols.
• Completes Face 2 Face, End of Life, and recertification assessment visits.
• Collaborates with other members of the health team to provide continuity of care.
• Documents and maintains data on all patients referred and seen.
• Participates in interdisciplinary team meetings and recommends course of treatment.
• Orders drugs and supplies, keeps records, and adjusts dosages as necessary.
• Assists family and patients regarding decisions, including goals and values, withdrawal and withholding of interventions, and non-beneficial interventions.
• Educates the patient and family regarding diagnosis of medical condition and prognosis.
• Refers patients to appropriate health facilities and community resources.
• Provides education to nursing staff and family members regarding the special needs of the patient and elderly population.
• Advises on appropriate action based on the patient’s condition.
• Independently orders IV therapy, including antibiotics and opioids, altering dosages and changing to other drugs as needed.
• Furnishes medications per standardized procedures, including those available under Providence St. Josephs Health formulary or those prescribed by specialty physicians.
• Assists in the placement/replacement of nasogastric or replacement of gastric tubes, adjusting rates and formulas based on patient needs.
• Communicates with physicians regarding patient care and outcomes.
• Participates in quality improvement activities related to hospice and palliative care.

Requirements

• Masters Degree in Nursing
• 2 years of experience in acute or transitional care setting

Qualifications

• Active and unencumbered RN license in the state of practice
• National Provider BLS - American Heart Association certification

Pay

$56.93 - $73.67 per hour

Schedule

Regular hours, with occasional on-call rotations.
