Responsibilities
- Carry out virtual onboarding E/M visits, including gathering patient history, conducting assessments, documenting findings, issuing necessary orders and referrals, and planning follow-up actions.
- Make clinical decisions between visits to ensure timely care and appropriate escalation, coordinating with primary care physicians and specialists to maintain continuity of care.
- Analyze RPM data, alerts, and trends to identify members who need outreach or escalation, documenting clinical interpretations and routing follow-up actions as required.
- Escalate clinical concerns to the Clinical Director and/or medical directors according to established protocols.
- Collaborate with care managers to develop, review, and approve individualized care plans aligned with evidence-based guidelines and member goals.
- Offer day-to-day clinical guidance to care navigators and care managers, addressing escalations and mentoring on documentation quality and escalation judgment.
- Contribute to refining clinical workflows, templates, and protocols to enhance safety, efficiency, and member experience.
- Identify recurring needs, outcomes, and team feedback to propose improvements and drive process optimization.
Requirements
- Active, unrestricted Florida NP license in good standing with at least 2 years of experience providing direct patient care, preferably in virtual care settings.
- Strong foundational knowledge of E/M (Evaluation and Management) including assessment, treatment planning, documentation, and order issuance.
- Experience managing chronic and/or complex conditions, demonstrating sound judgment on when more intensive evaluations are needed.
- Experience with telehealth, remote patient monitoring (RPM), care management, or care navigation is a plus.
- Exceptional communication skills, balancing empathy, clarity, and follow-through, with strong collaboration and escalation judgment within a clinical team.
- A high level of attention to detail, capable of managing multiple members' cases effectively and ensuring accurate documentation and follow-up.
- Proficiency in electronic health records (EHRs) and other digital tools, with the ability to quickly learn and adapt to new systems.
- Experience leading or mentoring clinical teams is a plus, reflecting a builder mindset and ownership mentality.
