Job Summary Our Valparaiso Porter-Starke location is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals. As a Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals of all ages. Responsibilities Identifies, evaluates, and assesses the mental health needs of clients, developing treatment plans and prescribing appropriate interventions and medications.Works with individuals, families, and groups to assess emotional, social, and physical health needs, consulting with general medical professionals as needed.Documents all diagnoses, ensuring accurate documentation of disease progression and manifestation.Seeks consultation and supervision in complicated cases, evaluating clients' responsiveness to treatment and recommending changes in treatment goals.Provides coaching, instructions, and direction to professional and paraprofessional staff, participating in medical staff meetings and committee assignments as directed. Education And Experience Current licensure and prescriptive practice as a Nurse Practitioner.Experience or training in mental health and/or chemical dependency treatment is required.
