Nurse Practitioner
About the role
Pine Park Health is seeking a Full-Time Nurse Practitioner to deliver care in our Phoenix, Arizona market. This role focuses on a smaller patient panel of complex care patients within a value-based care model, supported by a motivated multi-disciplinary team. Our person-centered care model leverages care navigators to maximize provider time and energy.
Responsibilities
- Provide onsite primary care services to older adults living in Senior Living Communities
- Develop and execute detailed person-centered care plans for all primary care patients
- Collaborate with the Medical Director to identify and develop policies addressing day-to-day practice, culture, or clinical guidelines, including emerging diseases
- Conduct monthly Chronic Care Management (CCM) visits, review, and sign off on CCM documentation
- Run regular huddles and provide leadership to the clinical team
- Identify ways to work with senior living management and care teams to improve patient outcomes and carry out Pine Park care plans
Qualifications
- Nurse Practitioner certification, preferably with geriatrics experience
- Comfortable working in a fast-paced and changing environment
- Willingness to provide onsite clinical care across the Central Phoenix/Tempe region
- Desire to work with complex care patients and support a holistic care model
- Strong orientation toward collaboration and working closely with a multidisciplinary team
- Excellent written and verbal communication skills
- Ability to work with tech-enabled care delivery tools (e.g., project management, checklist management, remote monitoring)
- Flexible work schedule with travel within the Phoenix area; driving between partner communities as needed
