Responsibilities
- Partner with the Interdisciplinary Team to complete key assessment sections, contribute to care planning, and support service determination requests.
- Actively build relationships with participants and manage an ongoing panel with continuity, accountability, and heart.
- Initiate, evaluate, review, and update medication and treatment orders (including diagnostic studies) with required approval.
- Assess, initiate, and coordinate day-to-day care under supervision—keeping participants stable, supported, and progressing toward goals.
- Document participant interactions, assessments, and next steps in the EMR within established center timeframes.
- Make referrals, request consults, and ensure follow-through with approval—helping participants navigate complex systems with ease.
- Perform histories and physicals, provide participant education, and support informed decision-making at every step.
- Order, interpret, and follow up on lab tests and results—closing loops and acting quickly when needed.
- Perform selected procedures within scope and support high-quality, efficient care delivery.
- Recognize abnormalities, respond with clinical judgment, and escalate concerns to ensure participant safety.
- Work closely with behavioral health, pharmacy, and other care partners to ensure wraparound, whole-person care.
- Evaluate and treat participants in the center, at home, virtually, in SNFs, and other settings—delivering care across the continuum.
- Facilitate advance care planning discussions with participants and caregivers with empathy and clarity.
- Document chronic conditions through HCC capture to reflect true clinical complexity and support appropriate care planning.
- Provide on-call coverage as needed, supporting timely guidance and continuity for participants.
Qualifications & Experience
- Lead with mission: Energized by serving high-risk seniors and frail older adults; bring empathy and purpose to work.
- Practice with proven NP experience: 2+ years of experience practicing as a Nurse Practitioner, ideally in settings serving medically complex and/or geriatric patients.
- Bring strong clinical depth: Hands-on experience caring for complex and older adult populations, comfortable navigating multiple chronic conditions.
- Communicate with clarity and warmth: Confident communicator building trust quickly with participants, caregivers, and teammates.
- Thrive with autonomy and teamwork: Can lead independently, make sound clinical decisions, and collaborate seamlessly within an interdisciplinary team.
- Grounded RN foundation: 2 years of RN experience supporting medically complex populations prior to becoming a Nurse Practitioner.
- Licensed and ready to practice: Active, unrestricted California NP license (and California RN license, as applicable) and current CPR certification.
- Strong educational preparation: Graduate of an accredited School of Nursing and hold a Master’s degree from an accredited NP (or PA) program.
- Credentialed for full-scope practice: Nurse Practitioner certificate with prescriptive authority, current on evidence-based standards.
- Geriatrics-informed and outcomes-driven: Up-to-date knowledge of chronic care and geriatric best practices, commitment to continuous learning.
- Bilingual skills are a plus: English required; Spanish preferred to better support the communities we serve.
- Mobile and community-ready: Valid California driver’s license, reliable transportation, good driving record, and auto insurance as required by state law.
