Nurse Practitioner

About the role

Longevity Health is seeking a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. This role involves supporting and demonstrating commitment to Longevity Health’s purpose: optimizing the quality of life of people in long-term care by focusing on their individualized health needs.

Key Tasks and Responsibilities

• Provides collaborative primary care and services to a panel of members
• Delivers ISNP Model of Care activities, develops and implements advanced care planning, and embraces/executes a treat in plan model to avoid unnecessary hospitalizations
• Facilitates and administers Health Plan clinical operations functions such as prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth
• Serves as a trusted advisor to members, loved ones, and nursing home staff and colleagues
• Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and interdisciplinary care team members on members’ care needs, goals of care, treatment plans/changes
• Serves as a key member of the nursing facility care team and ensures the care team stays aligned with members’ goals and needs
• Supports overall market goals/targets and balances member caseloads
• Promotes Longevity Health with members, loved ones, and nursing facilities and other stakeholders
• Functions autonomously and establishes meaningful relationships in the nursing home environment
• Is member-centric and customer service oriented, ensuring effective and high-quality care for Longevity Health members
• Performs other related duties as assigned

Requirements

• 6+ months experience as a Nurse Practitioner or Physician Assistant
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program
• Experience working in the nursing home environment is strongly desired
• Computer/Technology proficiency required
• The ability to engage and communicate effectively with physicians, members, and family members
• Demonstrated critical thinking skills, organization, and prioritization skills
• Credentials & Coverage:
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant
  • NCCPA board certified required
  • DEA License required
  • Must be eligible to participate in the Medicare program as a provider and registered
  • Valid state driver's license with a good driving record and proof of automobile insurance required
