Nurse Practitioner
About the role
Longevity Health is seeking a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. This role involves supporting and demonstrating commitment to Longevity Health’s purpose: optimizing the quality of life of people in long-term care by focusing on their individualized health needs.
Key Tasks and Responsibilities
- Provides collaborative primary care and services to a panel of members
- Delivers ISNP Model of Care activities, develops and implements advanced care planning, and embraces/executes a treat in plan model to avoid unnecessary hospitalizations
- Facilitates and administers Health Plan clinical operations functions such as prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth
- Serves as a trusted advisor to members, loved ones, and nursing home staff and colleagues
- Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and interdisciplinary care team members on members’ care needs, goals of care, treatment plans/changes
- Serves as a key member of the nursing facility care team and ensures the care team stays aligned with members’ goals and needs
- Supports overall market goals/targets and balances member caseloads
- Promotes Longevity Health with members, loved ones, and nursing facilities and other stakeholders
- Functions autonomously and establishes meaningful relationships in the nursing home environment
- Is member-centric and customer service oriented, ensuring effective and high-quality care for Longevity Health members
- Performs other related duties as assigned
Requirements
- 6+ months experience as a Nurse Practitioner or Physician Assistant
- Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program
- Experience working in the nursing home environment is strongly desired
- Computer/Technology proficiency required
- The ability to engage and communicate effectively with physicians, members, and family members
- Demonstrated critical thinking skills, organization, and prioritization skills
- Credentials & Coverage:
- State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant
- NCCPA board certified required
- DEA License required
- Must be eligible to participate in the Medicare program as a provider and registered
- Valid state driver's license with a good driving record and proof of automobile insurance required
