About the role

This role is for a Full-Time Advanced Practice Provider, either a nurse practitioner or physician assistant, providing primary and urgent care in a sub-acute, long-term care setting.

Responsibilities

• Work collaboratively with center leadership, medical directors, and healthcare team members to deliver high-quality care
• Support residents' health and well-being
• Provide comprehensive patient care
• Collaborate with medical teams
• Support center goals

Qualifications

• Graduate from an accredited nursing or PA program
• Licensed and certified accordingly
• Preferred experience working with adult or geriatric populations
• State licensure required
• Certifications such as NCCPA required
• New grads are welcome
