Key Responsibilities
- Provide direct palliative care, including comprehensive assessments, symptom management, and care planning in community settings
- Conduct history and physical exams, order and interpret diagnostics, and document findings accurately
- Develop and implement individualized care plans aligned with patient goals, values, and quality-of-life needs
- Collaborate with interdisciplinary teams, physicians, and caregivers to ensure coordinated and holistic care
- Facilitate advance care planning discussions, including goals of care and end-of-life decisions
- Support hospice services through face-to-face recertification visits and ensure compliance with regulatory requirements
- Work a schedule of four (4) 10-hour days per week
- Participate in on-call rotation one week every six weeks (telephone only)
- No in-person visits required during on-call periods
Requirements
- Current RN license
- Graduate of accredited nursing program
- Master’s degree with 1+ year NP experience and 1+ year in hospice/palliative care
- State-required advanced practice nursing certification
- Specialty certification preferred (CHPN, Pediatrics, or Geriatrics)
- Valid driver’s license and insured, reliable vehicle
