Duties & Responsibilities

• Transformative Member Engagement: Ignite change through dynamic 1:1 consultation and coaching sessions, employing motivational interviewing skills to build powerful, therapeutic connections.
• Empowering Health Assessment: Elevate members' wellness journeys with vibrant, advanced health assessments, offering guidance and education that sparks lasting transformations.
• Holistic Care and Impactful Management: Deliver standout routine care, blending standardized procedures with personalized touches, including physical examinations and screenings.
• Follow-up Excellence and Precision Documentation: Bring excitement to every member follow-up, ensuring meticulous, focused documentation that reflects their unique progress and needs.

Qualifications & Requirements

• California licensed Nurse Practitioner or Physician Assistant.
• Master’s degree from an accredited NP or PA program.
• DEA Registration Certificate.
• OIG Clearance and NPI number.
• Must be able to meet and receive a background check to include criminal records clearance.
