Nurse Practitioner

Responsibilities

  • Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits.
  • In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis.
  • Serving as the agency’s on-call provider, which includes advising nursing staff on symptom management, prescribing medications including controlled substances, and completing death certificates.
  • Serving as a clinical expert to nursing and other staff.
  • Participating in community education and outreach.

Requirements

  • Education, Skills & Experience:
  • MS Nurse Practitioner or DNP Doctorate Nurse Practitioner with NYS license and national certification.
  • Must hold a current NY State RN license.
  • DEA license to prescribe controlled substances in New York State.
  • Current State of New York driver’s license and automobile insurance.
  • Two (2) years of Nurse Practitioner experience, preferably in primary care or home health.
  • Strong preference will be given to candidates with hospice experience and certification.

Pay

$95,000 - $105,000 annually.

Schedule

M-F, 8:30am to 4:30pm. One (1) weekend on call per month. One (1) to two (2) holidays on-call per year.
