Nurse Practitioner
About the role
As a Nurse Practitioner at Guidestar Eldercare you will address the suffering of an aging population afflicted with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted‑living facilities. You will enjoy the autonomy of private practice while receiving an assigned caseload, corporate clinical leadership collaboration, and administrative support from in‑house scheduling, coding, and billing departments. You will work as part of a multidisciplinary team that actively promotes patient safety, functionality, and dignity.
Responsibilities
- Travel daily to assigned facilities in Eastern Kentucky.
- Provide initial evaluations, continuing therapy services, and cognitive testing.
- Document the need and extent of services for each patient.
- Create and maintain positive professional relationships with client facilities, patients, and their families.
- Function autonomously while effectively servicing a prescribed daily caseload.
- Utilize screening assessment tools and electronic medical record systems.
- Maintain accurate documentation and communication in line with rigorous clinical care protocols.
Qualifications
- Proof of state licensure.
- Master’s degree in a Nurse Practitioner specialty from an accredited university.
- Nurse Practitioner certification through ANCC or AANP.
- Minimum of 1 year clinical APRN experience preferred; newly licensed APRNs with a clinical background in geriatrics, adult medicine, long‑term care, palliative care, hospice, or home health will also be considered.
- Neurology training and/or experience preferred but not required.
- Experience with screening assessment tools.
- Hands‑on experience with Electronic Medical Record systems, Microsoft Office, and Apple products (iPad).
- Proven time‑management, strong interpersonal, and customer‑service skills.
- Ability to pass pre‑employment background check, drug screening, and complete credentialing process.
- COVID‑19 and flu vaccination required for all Guidestar employees.
