Description
This position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc. This position augments the Physician's data gathering abilities and assists in reaching decisions and instituting care plans for our patients. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management.
- The position will require travel to and from the 9 health centers operated by DVCH.
Duties
Duties Include But Are Not Limited To:
- Act as an advanced care practitioner and assist in coordinating all aspects of patient care.
- Responsible for rendering professional medical services to the patient population.
- Accountable for the complete documentation of patient visits in the patient chart and administrative reporting activities assigned by management.
- Provides health and wellness advice to patients or caregivers.
- Completes other duties as assigned.
Requirements
- A Registered Nurse Practitioner in the Commonwealth of Pennsylvania is required.
- Previous experience in Adult/Family Practice is highly preferred.
- Excellent interpersonal and oral and written communications skills are required.
- Bilingual (English/Spanish) highly preferred.

Base Salary is Board of Estimate (BOE)

Full-time, Monday - Friday
Qualifications
Qualifications, Education, and Knowledge:
- A Registered Nurse Practitioner in the Commonwealth of Pennsylvania is required.
- Previous experience in Adult/Family Practice is highly preferred.
