About the role
Community Healthcare Network is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually. We are seeking a Family Nurse Practitioner for our Washington Heights Health Center located at 511 West 157th Street, New York, NY 10032-5058.
Responsibilities
- Provide evidence-based Primary Care to patients
- Serve as an integral member of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health
Requirements
- Valid Family Nurse Practitioner N.Y.S. License to deliver medical care
- At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting
Schedule
- 35 hours per week (Full-time)
