About the Role

The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high-quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.

Responsibilities

  • Direct patient physical assessment, education, and health planning
  • Maintain continuity of care by regular patient follow-up
  • Assist in the coordination of services with other disciplines and patients' insurance plans
  • Advocate for patients in arranging medically necessary services
  • Communicate with community resources to ascertain adequate assistance for patients in need
  • Refer patients to specialty services when necessary
  • Consult with collaborating physician and other members of the health care team to establish plan of care
  • Participate in quality assurance process to assure optimal care delivery
  • Provide comprehensive case management services with emphasis on health promotion and disease prevention
  • Monitor patient outcomes in accordance with practice protocols
  • Collaborate with staff nurses and nursing administrators on assessment and clinical decision-making in patient care planning
  • Act as a resource person for the nursing staff
  • Conduct in-service education on practice issues
  • Perform other duties as assigned

Qualifications

  • Graduated from an approved Nurse Practitioner Program
  • Licensed as a Nurse Practitioner in New York State
  • Basic CPR (BCLS) certification required
  • At least 3 years of clinical experience as a primary care Nurse Practitioner
  • Board/Specialty Certification preferred
  • Bilingual in English and Mandarin Chinese required
