We are excited to announce an opportunity for a dedicated and compassionate Nurse Practitioner (NP) to join our team at Carease Health in Pelham Gardens, NY. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will be responsible for building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
Responsibilities
- Conduct on-site visits and remote patient monitoring to identify potential health issues early
- Provide chronic care management and psychiatric collaborative care to patients
- Document patient visits and treatment plans using our streamlined documentation system
- Collaborate with our clinical team to develop individualized care plans for each patient
- Participate in ongoing training and education to stay up-to-date on the latest best practices and guidelines
Requirements
- Active NP or PA license in the state of New York
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- Ability to work independently and as part of a team
