As a Nurse Practitioner (or Physician Assistant) you will deliver high‑quality preventative care to residents in skilled nursing facilities, working in a hybrid model that combines on‑site visits with remote work from home, supported by a dedicated scribe team.

Responsibilities

• Provide high‑quality preventative care to residents in skilled nursing facilities.
• Conduct on‑site visits and remote work from home, supported by a dedicated scribe team.
• Build strong relationships with facility staff and maintain a patient‑centered approach.
• Collaborate with other healthcare professionals to ensure seamless patient care.
• Maintain accurate and timely documentation of patient visits and treatment plans.

Requirements

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.
• Masters‑level education in nursing or a related field.

Compensation

$15,000 – $17,000 annually.

Schedule

Hybrid role with a combination of remote work from home and on‑site visits.
