About the role

Carease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.

Responsibilities

• Deliver high-quality preventative care to residents in skilled nursing facilities
• Build strong relationships with the facility
• Maintain a patient-centered approach
• Collaborate with other healthcare professionals

Requirements

• Active NP or PA license (in the state of practice)
• 1+ year of experience in:
• Geriatric care
• Documentation review
• Excellent communication, collaboration, and teamwork skills
