About the role

Carease Health is seeking an experienced Nurse Practitioner (NP) to join our team of dedicated healthcare professionals. As a Nurse Practitioner with our organization, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. With a hybrid work setup that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation, you will have the flexibility to balance your work and personal life.

Responsibilities

• Provide advanced nursing care to residents in skilled nursing facilities, with a focus on chronic disease management and prevention
• Conduct patient assessments, develop care plans, and provide medication management and education
• Collaborate with facility staff, families, and other healthcare professionals to ensure comprehensive care
• Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to proactively manage patient care
• Conduct on-site visits and remote work from home, with support from our dedicated scribe team
• Maintain accurate and timely documentation, ensuring seamless patient transitions and efficient facility communication
• Participate in quality improvement initiatives to enhance patient outcomes and facility performance

Requirements

• Active NP license (in the state of practice)
• 1+ year of experience in Geriatric care and/or Internal Medicine
• Excellent communication, collaboration, and teamwork skills
• Strong patient-centered approach and ability to build strong relationships with patients and facility staff
