Nurse Practitioner

Responsibilities

• Evaluate and treat patients in a team-based, value-driven primary care environment.
• Coordinate patient services, including referrals, hospital, SNF, DME, and home health care.
• Lead and participate in daily team huddles, fostering a collaborative practice.
• Complete timely medical documentation, optimizing coding and adhering to policies.
• Support quality improvement through outcome reviews and policy discussions.
• Assist in the growth of services and participate in on-call programs.
• Ensure compliance with licensing, certification, and accreditation standards.

Qualifications & Skills

• Bachelor’s degree and active, unrestricted Nurse Practitioner/PA license.
• Minimum of 8 years of technical experience and 2 years in project leadership.
• Passion for improving patient experience and participating in TB screening programs.
• Willingness to engage in limited on-call coverage.
• Preferred: Master’s in Nursing or PA certification.
