We're looking for a dedicated Nurse Practitioner to join our Program of All-Inclusive Care for the Elderly (PACE) team. If you're passionate about caring for older adults, enjoy building meaningful relationships with patients, and want to work alongside a collaborative interdisciplinary team, we'd love to meet you. As a Nurse Practitioner with PACE, you'll have the opportunity to provide comprehensive, patient-centered care to a vulnerable and deserving population. Rather than rushed visits and fragmented healthcare, you'll enjoy a more holistic approach that allows you to truly know your participants and impact their quality of life. You'll work closely with physicians, nurses, therapists, social workers, and other healthcare professionals to coordinate care that helps seniors remain independent and thrive in their communities.
