Nurse Practitioner
About the role
TruHealth is seeking an advanced practice provider—either a Nurse Practitioner (NP) or Physician Assistant (PA)—with experience in primary care or internal medicine, preferably with a geriatrics focus. This position offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. You’ll work primarily within nursing homes or assisted living communities, providing personalized, coordinated healthcare to residents enrolled in special Medicare Advantage plans. Your role will focus on enhancing quality of life and preventing unnecessary hospital visits while exercising independent judgment to treat patients with acute and chronic conditions.
Responsibilities
- Assess patients’ medical and mental health needs and develop proactive care plans that address conditions and barriers to care.
- Ensure compliance with local, state, and federal regulations related to clinical services.
- Prescribe medications, order lab work, diagnostic procedures, and consultations.
- Monitor patients’ compliance and response to treatment, modifying plans as needed.
- Collaborate with RN case managers as part of an integrated care team.
- Work with facility partners and health plan leadership to implement TruHealth’s national model of care.
- May provide training and advice to facility staff.
Requirements
- FNP, AGNP, or PA license required.
- Degree from an accredited APRN or PA program.
- 3+ years of experience in clinical nursing, rehabilitation, or geriatric populations.
- Experience with Electronic Health Records (EHR).
- Working knowledge of Microsoft applications (Word, Outlook, Excel).
