About the role
Albuquerque Health Care for the Homeless has been dedicated to serving people experiencing homelessness since 1985. Our mission is to provide a continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness.
Responsibilities
- Provide chronic disease management and substance use disorder treatment within an integrated care model.
- Collaborate with other departments and agencies to offer comprehensive care.
- Treat clients in both the clinic and mobile teams, adhering to a treat-first model.
- Work with clients in a shared client model, sharing clients with multiple practitioners.
- Offer walk-in and scheduled appointment hours, with no on-call or weekend hours.
Requirements
- Master's degree in Nursing
- CNP license through the NM Board of Nursing
- Current DEA license
- Current CSR license
Preferred Qualifications
- 2 years of experience providing medical care in primary or family practice setting
- 2 years experience in Federally Qualified Health Center or in public health
- Experience treating co-morbid conditions
- Experience with eCW EHR
- Knowledge of harm reduction and trauma informed philosophies
- Previous work with under-served populations or people experiencing homelessness

Qualifications
Compassion for and an understanding of the wide variety of issues for those experiencing homelessness is a must. Trauma is an underlying issue with most of our clients and we need a candidate that has a working knowledge of trauma informed care. Our clinic operates in a shared client model so the ideal candidate would be comfortable sharing clients with multiple practitioners.
Schedule
We have no on-call or weekend hours. Our team is very passionate about public health and our population.
