Nurse Practitioner

Job Description

Counsels and educates patients and families regarding treatment plans and health management. Coordinates patient care with physicians and interdisciplinary team members. Implements and evaluates individualized plans of care, considering age-specific needs. Manages acute and chronic health conditions across organ systems. Facilitates efficient and effective patient flow, including appropriate utilization of resources and discharge planning. Participates in interdisciplinary rounds, case conferences, and care planning activities.

• Responds to urgent or emergent patient needs and participates in on-call responsibilities as scheduled.
• Engages in professional development activities and represents the organization in appropriate community or educational events.
• Performs other duties as assigned.

• 1+ year of related work experience [Required]

• Advanced Practice Registered Nurse (APRN) [Required]
• Advanced Cardiac Life Support (ACLS) [Required]
• Basic Life Support – CPR Cert (BLS) [Required]
