About the role

The Nurse Practitioner provides medical and psychiatric evaluation, assessment, medication management, care coordination, and ongoing treatment services for individuals receiving behavioral health and substance use disorder treatment. This role collaborates with interdisciplinary treatment teams and internal and external healthcare providers to support high-quality, person-centered care in accordance with organizational, regulatory, and professional practice standards.

Responsibilities

  • Complete history and physical assessments, evaluations, reassessments, and other clinical documentation as required.
  • Review laboratory results, monitor blood levels, review physician orders, and support ongoing medical and medication management needs.
  • Provide medication management services, including psychotropic medication management where applicable based on licensure and scope of practice.
  • Diagnose and treat medical and/or psychiatric conditions in accordance with scope of practice and applicable regulations.
  • Coordinate client care with internal and external healthcare providers, including primary care physicians, psychiatrists, specialists, pharmacies, and other treatment providers as appropriate.
  • Support discharge planning and continuity of care efforts, including medication coordination and aftercare recommendations.
  • Collaborate with nursing and clinical teams to support cost-effective medication management and completion of medication prior authorizations when needed.
  • Provide education and clinical support to staff regarding medical, psychiatric, behavioral health, and medication-related concerns as appropriate.
  • Participate in treatment team meetings, clinical review meetings, peer review processes, and departmental meetings as required.
  • Maintain timely, accurate, and compliant documentation in accordance with CARF standards, licensing requirements, payer requirements, and organizational policies.
  • Exercise discretion and maintain confidentiality with regard to all company information.
  • Complete all required trainings as designated by the company and accreditation/licensing entities.
  • Perform other duties as assigned.

Requirements

  • PMHNP or Nurse Practitioner license in New Jersey.
  • Minimum of 2 years’ experience in addiction medicine and/or family practice medicine.

Schedule

30 hours per week.

Pay

$84.00 per hour.
