About the role

The Nurse Patient Navigator at Children’s Hospital of Richmond at VCU is a key member of the interdisciplinary team, providing advocacy, education, and counseling to patients and their families. This role requires a strong foundation in nursing, with a focus on patient care coordination and support.

Responsibilities

  • Serves as a single point of contact for assigned patients and caregivers, ensuring they have access to necessary resources and information.
  • Collaborates with a multidisciplinary team to manage the care of assigned patients, focusing on comprehensive and high-quality healthcare.
  • Interacts with patients and caregivers to interpret medical information and provide guidance on treatment options and recovery processes.
  • Assists patients and caregivers in navigating the healthcare system, addressing any barriers to care and advocating for their needs.
  • Monitors patient progress and communicates with healthcare providers to ensure continuity of care.
  • Provides emotional support and counseling to patients and caregivers during challenging times.

Requirements

  • Licensure, Certification, or Registration Requirements: Current RN licensure in Virginia or eligible compact state. AHA BLS Certification. Specialty Nursing Certification preferred within one year of hire.
  • Experience: Minimum of three (3) years of relevant clinical experience in nursing, patient education, case management, or navigation. Previous experience in nurse navigation or leadership preferred.
  • Education: Baccalaureate Degree in Nursing from an accredited School of Nursing. Master’s Degree in Nursing preferred.
