About the role
The Nurse Patient Navigator at Children’s Hospital of Richmond at VCU plays a crucial role in advocating, interpreting, educating, and counseling patients and their families. They serve as a key member of a multidisciplinary team, ensuring that patients receive comprehensive and high-quality care.
Responsibilities
- Serves as the primary point of contact for assigned patients and caregivers, providing support throughout their treatment journey.
- Collaborates with a multidisciplinary team to manage the care of assigned patients, ensuring continuity and coordination of care.
- Uses clinical and nursing knowledge to guide patient care and provide educational resources.
- Interacts with patients and families to interpret medical information and facilitate communication between patients, families, and healthcare providers.
- Assists patients and families in navigating the healthcare system and addressing any barriers to care.
- Monitors patient progress and communicates changes in condition to healthcare providers.
- Ensures that patients and families have access to necessary resources and support services.
Requirements
- Licensure, Certification, or Registration Requirements: Current RN licensure in Virginia or eligible compact state. AHA BLS Certification. Specialty Nursing Certification in Nurse Navigation or area of specialty within one year of hire.
- Experience: Minimum of three (3) years of relevant clinical experience in nursing, patient education, case management, or navigation. Previous experience in nurse navigation or leadership preferred.
- Education: Baccalaureate Degree in Nursing from an accredited School of Nursing. Master’s Degree in Nursing from an accredited School of Nursing preferred.
