Responsibilities
- Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
- Conduct telephonic patient assessments using evidence-based protocols to help patients navigate the complex healthcare system.
- Provide health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
- Engage in telemonitoring activities using cutting-edge tools to track progress and intervene proactively.
- Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
- Represent GMR’s innovative programs within the community, reaching populations that need care most but often go underserved.
- Serve as a facilitator and educator for GMR’s integrated healthcare training, empowering others to deliver better care.
- Manage time, resources, and documentation efficiently while providing outstanding customer service and support.
Requirements
- Valid RN Compact State License.
- Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment.
- Experience with clinical data collection, ethical principles, and patient advocacy.
- Graduate of an accredited nursing program.
- Current RN license in a compact state.
- Current CPR certification.
Qualifications (Preferred)
- Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields, especially with a focus on community health or disease education.
Pay
$41.80 - $46.75 per hour.
