Responsibilities

• Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
• Conduct telephonic patient assessments using evidence-based protocols to help patients navigate the complex healthcare system.
• Provide health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
• Engage in telemonitoring activities using cutting-edge tools to track progress and intervene proactively.
• Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
• Represent GMR’s innovative programs within the community, reaching populations that need care most but often go underserved.
• Serve as a facilitator and educator for GMR’s integrated healthcare training, empowering others to deliver better care.
• Manage time, resources, and documentation efficiently while providing outstanding customer service and support.

Requirements

• Valid RN Compact State License.
• Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment.
• Experience with clinical data collection, ethical principles, and patient advocacy.
• Graduate of an accredited nursing program.
• Current RN license in a compact state.
• Current CPR certification.

Qualifications (Preferred)

• Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields, especially with a focus on community health or disease education.

Pay

$41.80 - $46.75 per hour.
