Nurse Navigator RN
Starting pay: $41.80 - $46.75 per hour. Night shifts available. On-site in Phoenix, AZ. No direct patient care; 12-hour rotating shifts including weekends.
About the Role
Join an innovative healthcare team as a Nurse Navigator – Registered Nurse (RN). This role focuses on connecting resources, leveraging technology, and delivering care to underserved populations without direct bedside care. You’ll coordinate care, guide patients through their health journeys, and be at the forefront of a healthcare revolution using technology and teamwork to create lasting impact.
Responsibilities
- Use nursing expertise to connect mobile healthcare resources and integrate services to improve patient outcomes.
- Conduct telephonic patient assessments using evidence-based protocols to help patients navigate the healthcare system.
- Provide health education, support healthy behaviors, and help patients understand their care options.
- Engage in telemonitoring activities using cutting-edge tools to track progress and intervene proactively.
- Lead and collaborate in team care planning, mentor colleagues, and champion new approaches in healthcare delivery.
- Represent GMR’s innovative programs within the community, reaching underserved populations.
- Serve as a facilitator and educator for integrated healthcare training.
- Manage time, resources, and documentation efficiently while providing outstanding customer service.
Requirements
- Valid RN Compact State License.
- Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment.
- Experience with clinical data collection, ethical principles, and patient advocacy.
- Graduate of an accredited nursing program.
- Current CPR certification.
- Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields is a plus (especially if focused on community health or disease education).
