Nurse Navigator LPN - PRN

Responsibilities

• Educate patients with a recommendation for a skilled nursing or rehabilitation on their post-acute options.
• Discuss quality analytics for skilled nursing facilities with patients.
• Anticipate and address barriers to discharge through critical thinking and collaboration with an interdisciplinary team.
• Communicate effectively with patients, families, post-acute care facilities, and the interdisciplinary team.
• Maintain productivity by managing numerous priorities and remaining organized.

Requirements

• Less than one year related experience.
• High School/GED education.
• Licensed Practical Nurse - Current Maryland license or eligibility to obtain Maryland license.
